Your first visit to Christopher H. Berkers, DMD establishes a vital foundation for our relationship with you. During this visit, Dr. Berkers will take time to listen to your concerns and expectations. We make sure to obtain important background information, like your dental and medical history. Your initial oral exam includes a visual examination, oral cancer screening, charting, periodontal probing, and diagnosis and treatment recommendations. We will take x-ray for proper diagnosis, including bitewing x-rays for detection of decay between the teeth.
To understand what to expect for your first visit to our practice, please read our website. You’ll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. There is also background about our committed staff and first visit procedures. You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing the content on this website. Also, take some time to review our staff page and familiarize yourself with the doctors. We look forward to your first visit.
Please print and fill out these forms so we can expedite your first visit:
In order to view or print these forms, you will need Adobe Acrobat Reader installed.
Due to an unfortunate and dramatic rise in NO CALL/ NO SHOW/LATE CANCELLATIONS of appointments we will be enforcing our existing policy of charging $50.
This is not something we like to do and have been able to be lenient in the past. However, the dramatic rise is affecting our ability to provide care in a timely manner to the patients that are able to commit to their appointment times.
We appreciate your attention to this change.
